Shirley A. Conibear MD, MPH, FACPM
President

Dr. Conibear is a frequently published and renowned board-certified occupational medicine physician with over 35 years of experience. She has served as Consulting Medical Director to such firms as Milwaukee Road, Morton Chemical, ICI Paints, Waste Management, DAP, USG Corp and Akzo Nobel.  In the clinic, she performs and oversees comprehensive patient examinations that evaluate fitness-for-duty, short and long term disability and workers' compensation issues as well as US Civil Service Immigration and OSHA compliance physicals.

Dr. Conibear is Board Certified in Occupational Medicine and residency trained in Family Medicine. She holds a Medical Doctor (MD) and Masters of Public Health (MPH) degree from the University of Illinois, where she is currently an Adjunct Associate Professor.  She teaches and supervises residents in Occupational Medicine during their practicum at the OMS.

Dr. Conibear is:

  • A certified Medical Review Officer (MRO) in good standing
  • A Designated Civil Surgeon for the US Citizenship and Immigration Services
  • Fellow in College of Preventative Mid- Occupational Medicine
  • Licensed to practice medicine in 16 states

Virginia A. Evans, MD, MPH, FACEP
Director, Clinical Services – Occupational Medicine Physician

Dr. Evans is Board Certified in Occupational and Environmental Medicine, and prior to joining The OMS, Ltd., practiced as a Board Certified Emergency Medicine Physician in Cleveland, Ohio for over thirty years. She holds a Medical Doctor (MD) degree from Case Western Reserve University in Cleveland, OH. She also holds a Masters of Public Health (MPH) degree from the University of Illinois – Chicago, where she continues to serve as an Adjunct Assistant Clinical Professor in the UIC Occupational and Environmental Medicine Residency Program.

In the clinic, she performs and oversees comprehensive patient examinations that evaluate fitness-for-duty, short and long term disability and workers’ compensation issues as well as US Civil Service Immigration and OSHA compliance physicals. Dr. Evans is also certified as a Federal Aviation Administration (FAA) Senior Aviation Medical Examiner (Senior AME).

Dr. Evans is licensed to practice medicine in 12 states

Jeanne Boysen, SPHR, SHRM-SCP
Vice President

Ms. Boysen manages the operations of The OMS, Ltd. and brings to OMS more than 20 years of experience in human resources and 5 years of experience as a paralegal.  Ms. Boysen’s training and experience provide a uniquely valuable ability to manage all clinic operations.  Ms. Boysen supervises medical and administrative assistants in the clinic to afford quality and accuracy in all OMS work product.  In addition to her SPHR and SHRM certifications, Ms. Boysen serves as Vice President of the Chicago chapter of the Disability Management Employer Coalition (DMEC) and has a Bachelor of Arts degree in Psychology from the University of Denver.  Ms. Boysen is also a DATIA Certified Professional Collector.

Ashley Wyatt
Medical Assistant

Ms. Wyatt plays an integral part in the total operation of the OMS clinic. She is a primary contact for scheduling, billing, and client communications. Ms. Wyatt is responsible for scheduling doctor’s examinations, DOT and OSHA compliance examinations, drug screens, respirator wearer physicals and respirator fit testing procedures both in the OMS clinic and at the client's work-site. Ms. Wyatt ensures quality and accuracy in maintaining patient test results, HIPAA confidential medical records, data entry and record storage. Ms. Wyatt ensures maintenance, certification and inspection occurs of all lab and medical test equipment, such as x-ray, EKG, vision, audiometer, PFT (spirometry), neurometer plus ancillary items. Ms. Wyatt is certified in CPR and has completed Audiometric Testing training and NIOSH-approved Spirometry training.

Tonya Brookshire
Administrative Assistant

Tonya Brookshire is the administrative assistant to the physicians and staff of OMS. Ms. Brookshire joined OMS in 2016, bringing more than 10 years of experience as an administrative assistant. She graduated from Northeastern Illinois University with a Bachelor of Arts degree in English Language and Literature. Ms. Brookshire performs clerical functions for a variety of tasks, including IMEs, FMLA asset protection reports, transcription of medical reports, Internet searches on medical topics, and coordinates the physician license program. She also is responsible for maintaining our case databases. Ms. Brookshire plays an integral part in the total operation of the OMS clinic. She assists with scheduling, billing, and client communications. She is a DATIA Certified Professional Collector and serves as the backup for the medical assistants.

Raja K. Khuri, M.D., MPH
 

Dr. Raja K. Khuri is board certified in Internal Medicine and Occupational Medicine. He served his Internal Medicine residency at Northwestern University and OM at University of Illinois. He earned an (MD) Medical Doctor degree from the American University of Beirut, Lebanon and an (MPH), Masters of Public Health degree from the University of Illinois where he also held an Assistant Clinical Professor appointment (emeritus). Dr. Khuri has over forty years of clinical practice in both fields and has served as Medical Director of Employee and Student health Service at Northwestern Memorial Hospital and Medical Director for Burlington Northern Santa Fe Railroad.

He was employed for 10 years at the OMS. He is a Certified Independent Medical Examiner (CIME) and a Certified Medical Review Officer (MRO). Dr. Khuri has performed comprehensive hands-on physicals for DOT, FRA, and OSHA compliance; executive exams, pre/post-employment exams, fitness-for-duty exams, and disability and workers’ compensation exams. He also was appointed by the American College of Occupational and Environmental Medicine (ACOEM) to its committee on Drug and Alcohol Abuse. 

Mohamad Farris, M.D., MPH
 

Dr. Mohamad Farris is residency trained and Board Certified in Occupational Medicine. He holds a Medical Doctor (MD) degree from American University of Antigua-College of Medicine. He also holds a Masters of Public Health (MPH) degree from the University of Illinois – Chicago. He is a Certified Medical Review Officer (MRO).

Dr. Farris has clinical experience in Occupational Medicine. He currently provides work-related health care, commercial driver’s (DOT) examinations, return to work physicals, fitness for duty physicals, as well as pre-employment physicals and OSHA compliance physicals. Dr. Farris is licensed to practice medicine in Ohio. 

Amy E. Allegretti, M.D., MPH, FACEP
Occupational Medicine Physician

Dr. Amy Allegretti is Board Certified in both Occupational Medicine and Emergency Medicine. She received her medical degree (MD) from Indiana University School of Medicine and a Master of Public Health (MPH) degree from the University of Illinois at Chicago. She currently has a faculty appointment at the University of Illinois at Chicago and teaches occupational medicine residents who rotate through OMS as part of their training program.

Dr. Allegretti has extensive clinical experience in both Emergency Medicine and Occupational Medicine. She currently performs and oversees comprehensive patient exams to evaluate fitness-for-duty, disability and workers’ compensation issues, and DOT and OSHA compliance. She is also a Certified Medical Review Officer (MRO) in good standing. Dr. Allegretti is licensed to practice medicine in Illinois, Ohio, California, and New York.
