Shirley A. Conibear MD, MPH, FACPM
President

Dr. Conibear is a frequently published and renowned board-certified occupational medicine physician with over 35 years of experience. She has served as Consulting Medical Director to such firms as Milwaukee Road, Morton Chemical, ICI Paints, Waste Management, DAP, USG Corp and Akzo Nobel.  In the clinic, she performs and oversees comprehensive patient examinations that evaluate fitness-for-duty, short and long term disability and workers' compensation issues as well as US Civil Service Immigration and OSHA compliance physicals.

Dr. Conibear developed a Family Medical Leave Act (FMLA) clarification program which, when implemented, has greatly reduced fraud and abuse as a cost containment service for our clients.

Dr. Conibear is Board Certified in Occupational Medicine and residency trained in Family Medicine. She holds a Medical Doctor (MD) and Masters of Public Health (MPH) degree from the University of Illinois, where she is currently an Adjunct Associate Professor.  She teaches and supervises residents in Occupational Medicine during their practicum at the OMS.

Dr. Conibear is:

  • A Designated Civil Surgeon for the US Citizenship and Immigration Services
  • CAOHC Certified Professional Supervisor
  • Licensed to practice medicine in 16 states

Raja K. Khuri, MD, MPH, FACPM
Director, Clinical Services

Dr. Khuri has extensive experience in occupational medicine, having served as Medical Director for Wesley Memorial Hospital for nine years and for the Santa Fe Railroad (later named Burlington Northern Santa Fe) for 18 years. During his tenure at Santa Fe, Dr. Khuri developed an occupational health department, hearing conservation program, drug testing program, disability management program, industrial hygiene program, an employee assistance program, and executive health program.

His skills in occupational and internal medicine add depth to the OMS staff.  Dr. Khuri is Board Certified in Internal Medicine, Board-Certified in Occupational Medicine, and holds a Masters of Public Health (MPH) degree from the University of Illinois. 

Dr. Khuri performs comprehensive physicals examinations for OSHA compliance, executive exams, pre/post employment exams, fitness-for-duty exams, and disability and workers' compensation exams. He also serves on our staff of Family Medical Leave Act (FMLA) clarification physicians for OMS clients. 

His past appointments include American College of Occupational and Environmental Medicine (ACOEM) Committee on Drug and Alcohol Abuse.

Dr. Khuri is currently an Adjunct Associate Professor at the University of Illinois School of Public Health where he teaches and supervises second and third year residents in Occupational Medicine.

Teresa Henley, AAS, CMA, CPCT, LRT
Manager, Medical Services

Ms. Henley plays an integral part in the total operation of the OMS clinic. She is the primary contact in all scheduling and communications with clients. From scheduling to billing, she is responsible for staffing and scheduling doctor’s exams, Immigration and OSHA compliance exams, drug screens, respirator-wearer physicals and fit testing procedures both in the OMS clinic and at the client's work-site.

She supervises and trains medical assistants to afford quality and accuracy in maintaining patient test results, confidential medical records, data entry and storage. Ms. Henley oversees maintenance, certification and inspection of all lab and medical test equipment, such as x-ray, EKG, vision, and audiometry. She is a Certified Medical Assistant (CMA) with an Associate Degree from Robert Morris College.

Jeanne Boysen, SPHR, CPCT

Ms. Boysen brings to OMS fifteen years of experience as Human Resources Director for a well-established consulting firm in downtown Chicago, and prior to that, years of experience as a paralegal for a variety of specialty law firms. Ms. Boysen's training and experience as a Certified Human Resource Professional provides a uniquely valuable ability to orchestrate the concerns of the various parties in the FMLA process with both the employer and the employee. Ms. Boysen brings exclusive talents to OMS in coordinating IME projects, QA/QC, follow-up, and consulting with OMS staff.  She has extensive experience with hundreds of FMLA second and third opinions. In addition to her SPHR certification, Ms. Boysen has a Bachelor of Arts degree in Psychology from the University of Denver.

Kelly Nelson, CMA
Certified Medical Assistant

Ms. Nelson is one of OMS’s key medical assistants.  She has over ten years of hospital, urgent care and clinical experience.  Ms. Nelson is fluent in all facets of Drug Screening, Immigration and OSHA compliance exams and testing from pre/post-employment physical profiles through comprehensive workers’ compensation and executive physical exams.

Nancy Eichendorf
Medical Secretary/Legal Assistant

Nancy Eichendorf is a medical secretary/legal assistant to the physicians and staff of OMS.  Ms. Eichendorf provides administrative support and coordinates FMLA certifications and second and third opinions for our clients.  She also is responsible for maintaining our client and provider databases and is proficient in medical record management.  Ms. Eichendorf is a Certified Professional Collector (CPC).
